client not allowed to relay - Where else do I need to check

We added a new user to Qmail and I'm getting the error that they are not allowed to send outside of our domain. I've added other users in the past without a problem everyone I am adding now cannot send outside. Everything is set the same between the users shown below. Any help would be greatly appreciated.

Note: in the second example of the log file the from is different <yyy@jacklighter.com::> than the first <xxx@jacklighter.com:xxx@jacklighter.com:> where does the second parameter come from and is that the problem and how do I fix it both users are using Outlook 2007
